Charles Cusack Jr.

DELAND - Charles Allen “Chuck” Cusack Jr., 67, died Nov. 13, 2009, in Knoxville, Tenn. Calling hours will be 4-6 p.m. Friday, Nov. 20, at Greater Union First Baptist Church, 240 S. Clara Ave. in DeLand. A wake will be at 6 p.m. Nov. 20 at Greater Union First Baptist Church. A funeral service will be at 10 a.m. Saturday, Nov. 21, at First Baptist Church of DeLand, 725 N. Woodland Blvd. Interment will be at Union Cemetery, 1600 W. Euclid Ave. in DeLand.
Mr. Cusack was born Dec. 21, 1941, in DeLand to the late Charles Allen Sr. and Ruth Evelyn Johnson Cusack. He attended the local schools and graduated from Euclid High School, Class of 1959. He matriculated to Florida Agricultural & Mechanical University (FAMU) in Tallahassee, receiving his bachelor’s degree in 1970. He later furthered his education at Nova Southeastern University, receiving his master’s degree in 1985.
At age 14, he also began working as groundskeeper for Harry Thomson, working for him until Mr. Thomson’s death in July 2004. He worked at Keller Kitchen Cabinets prior to college.
Mr. Cusack began his career in education in 1970 at Ormond Beach Elementary School, then continued at Woodward Avenue Elementary School in DeLand, and finally at DeLand Middle School, retiring in 2003. Throughout his 33 years of teaching, he served in sixth-grade classrooms. He also served as summer-school principal. In addition to the daytime classroom, he began teaching in the Alternative Education Program at Daytona Beach Community College in 1978, and assisted hundreds of students who returned to school to earn their high-school diploma.
He was converted and baptized at the age of 12 at Greater Union First Baptist Church in DeLand. He served as Sunday-school teacher, and in the Sanctuary and Inspirational Choirs, and the Men of Union Choir. He was ordained a deacon in 1984, a very proud moment in his life. As a deacon, he served as the leader of District No. 3. Most recently, he served as the pastor’s anniversary chairman. He served in many capacities, wherever he could be of assistance, including helping many of the senior members of the congregation with their personal needs and chauffeuring them as well as “Miss Joyce.”
Chuck was a member of Kappa Alpha Psi Fraternity Inc., Athens Lodge No. 46, Prince Hall Masons, and Progressive Pallbearers Lodge No. 119, and a Past Worthy Patron for Prince of Peace Chapter 120, Order of the Eastern Star. He served as treasurer for the West Volusia Branch of the National Association for the Advancement of Colored People for more than 25 years, and was a charter member of the Spring Hill Kiwanis Club, the Volusia County Sickle Cell Association, Family Renew and the FAMU Alumni Association.
On Dec. 26, 1961, Mr. Cusack married his high-school sweetheart, Joyce M. Cummings. To this union were born two daughters, Jan and Julie.
He leaves to cherish his memory his loving wife of 48 years, Joyce C. Cusack; two daughters, Jan E. Cusack-Johnson of DeLand, and Julie E. Cusack-Gordon and her husband, Vernon; three granddaughters, Danisha L. Johnson, Jada Gordon and Jade Gordon; one brother, James E. and his wife, Brenda, of DeLand; his namesake, nephew Charles Allen Cusack Ill and his wife, Karen, of DeLand; one godsister, Marilyn Hinson-Jordan; five aunts, Theresa Worthy and Willie Mae Johnson, both of DeLand, Hazel Cusack of Nashville, Ga., Florence P. Turner and her husband, Leroy, of Pomona Park, and Annie Bell of New Smyrna Beach; his mother-in-law and father-in-law, Gussie and Nathan Francis of DeLand; two sisters-in-law, Joan Lane and her husband, David, and Deletta Ebbensen and her husband, Vincent; nieces and nephews Thunnan Thomas, Deirdre Footman and her husband, Darron, Delvin Ebbensen, Deondra Ebbensen and DeRyan Ebbensen; and a host of God’s children, cousins, other relatives and friends.

Mrs. Forrer owned and operated many businesses, and was a musician.

DELTONA - Mark Francis Sr., 44, died Nov. 8, 2009, at his home.
Mr. Francis was the manager of Big T Tire in Sanford.
He graduated from Middletown High School in Rhode Island, where he was on the wrestling team.
Mr. Francis loved riding his motorcycle, as well as working on motorcycles. He was known as a great mechanic.
He was preceded in death by one stepbrother, Wayne Burgess. He is survived by his wife, Diana; one son, Mark; his father and stepmother, Rick Sr. and Linda; his mother, Jacqueline Francis; one sister, Catherine; two brothers, Rick Francis and his wife, Missy, and Bill Francis and his wife, Anna, all of Middletown; two stepbrothers, Larry Burgess and his wife, Sue, of Newport, R.I., and George Burgess and his wife, Nancy, of West Virginia; and 10 nieces and nephews, Cotter, Nichole and Katie, all of Rhode Island, Tyler, Cassie, Chelsey, George and Alexis, all of West Virginia, and Brooklynn and Shye, both of Florida.

OCALA - Dorothy Louise Grow, 78, a former DeLand resident, died Nov. 13, 2009.
Mrs. Grow retired as an inspector for Sherwood Medical Industries in DeLand after 25 years. Born in Tampa, she moved to Ocala in 2005 from Brevard, N.C.
She was a member of Little River Baptist Church in Little River, N.C.
Mrs. Grow is survived by two daughters, Gail L. Merrill and her husband, Al, of Ocala, and Katherine L. Collums and her husband, Marty, of Dunnellon; one sister, Ella Mae Adams of Brandon; two brothers, Richard McMillan of Riverview, and Thomas McMillan of Cottonwood, Ariz.; two grandsons, Robbie Merrill and Brett Collums; two granddaughters, Deanna Ladley and Lindsay Fleming; and seven great-grandchildren.

DELAND - Eloise Hodges, 95, died Nov. 13, 2009. A funeral service was held Nov. 16 at First Christian Church (Disciples of Christ) of DeLand. Interment was at Oakdale Cemetery.
Mrs. Hodges was an avid golfer at DeLand Country Club and Southridge Country Club, and was one of the oldest members of First Christian Church (Disciples of Christ) of DeLand.

GREER, S.C. - John Edwin Johns Jr., 58, a former resident of DeLand, died Nov. 18, 2009. The Rev. Aaron Rayburn will officiate at a funeral service at 2 p.m. Friday, Nov. 20, at Edwards Road Baptist Church, 1050 Edwards Road in Greenville, S.C. Interment will be at Woodlawn Memorial Park.
Born in DeLand, Mr. Johns was a vice president at Merrill Lynch, having served with the company for more than 25 years.
He was a graduate of Stetson University.
Mr. Johns was a member of Edwards Road Baptist Church, the Thornblade Club, and the Poinsett Club, and was an avid golfer.
He was preceded in death by his father, Dr. John E. Johns Sr. He is survived by his wife, Tracey Shealy Johns; his mother, Martha Mauney Johns of Greenville; one daughter, Lauren Ashley Johns of Greenville; two sons, Evan Thomas and William Mauney, both of Greenville; and two brothers, Steven Maxwell and his wife, Norine, of Simpsonville, S.C., and Marcus Mauney of Lilburn, Ga.

PORT ORANGE - Jake J. Van Dyk, 79, a former DeLand resident for more than 20 years, slipped away into the arms of Jesus Nov. 9, 2009, and is now rejoicing in His presence. A celebration of Jake’s life will be at 1 p.m. Saturday, Nov. 21, at Sterling House, 955 Village Trail in Port Orange.
Jake is survived by his wife of 55 years, Cornelia; two daughters, Corry Alexander and her husband, Doug, and Edith Rosema and her husband, Bruce; and his cherished granddaughters, Rachel Alexander and Bethany Alexander.
Jake’s journey began in Amsterdam on Aug. 27, 1930, with a family of five brothers and one sister. He was immensely proud to become an American citizen in 1972. His fulfilled life, enriched by faith, family and friends, spanned his years in Michigan, and the last 25 years in Florida.
Jake had a genuine love and commitment for his new country, wholeheartedly serving many years as a precinct clerk for Volusia County Elections. He also assisted the Port Orange community as a dedicated YMCA Ambassador. Jake’s spirit to enjoy life was evident in that “no one was a stranger,” resulting in a vast array of friendships across the country. His assurance in the Lord was manifested through Psalm 91:1-2.
The family expresses their gratitude to Sterling House for their loving care and respectful treatment during his last four weeks.

Mr. Knopf retired as a printer.

ORANGE CITY - Michael W. Polgar, 75, died Nov. 9, 2009, at his home.
Mr. Polgar was a logistic supervisor in the engineering industry. Born in New York City, he moved to Orange City 25 years ago from Nescopeck, Pa.
He was a member of St. Ann’s Catholic Church in DeBary, and was a political activist in Orange City. He loved to travel and spend time with his family.
Mr. Polgar served in the U.S. Army.
He is survived by his wife of 42 years, Olga; two sons, Rick of Stratford, N.Y., and Michael of Las Vegas; three daughters, Tammy Polgar and Michelle Polgar, both of Orange City, and Lilly Efird of DeLeon Springs; one sister, Helene Madl of Deltona; seven grandchildren and two great-grandchildren.

Mr. Eaton was an air-conditioning service technician.
Motorcycles were his life; he enjoyed restoring and painting them, and especially riding them.
Mr. Eaton served in the U.S. Army.

Mr. Bailey retired in 1980 as a service manager for a car dealership. Born in Silver Creek, N.Y., he moved to DeBary 29 years ago from Belfast, N.Y.

CHICKAMAUGA, GA. - Martha Marie Felix, 86, a former longtime resident of DeLand, died Oct. 27, 2009, at her home. A funeral service was held Nov. 3 at Deltona Memorial Funeral Home. Burial followed at DeLand Memorial Gardens.
Born in Huddy County, Ky., Mrs. Felix retired from environmental services at West Volusia Memorial Hospital.
She was a member of Southside Baptist Church in DeLand, and loved her family.
Mrs. Felix was preceded in death in 1997 by her husband, Harold. She is survived by two sons, Harold “Hal” of Browns Mills, N.J., and Gene of Deltona; two daughters, Marie Felix of Chickamauga, and Sandra Kleinschmidt of DeLand; two stepdaughters, Lois Herron and Zoe Faircloth, both of Jacksonville; one sister, Lucy Wyatt of Ohio; and many devoted grandchildren and great-grandchildren.

DELAND - Clay Raymond Mullins, 69, died Oct. 29, 2009, at his home. A funeral service was held Nov. 3 at the Church of Jesus Christ of Latter-day Saints in DeLand. Burial was at Sodus Rural Cemetery in Sodus, N.Y.
Mr. Mullins retired as an engineer for IBM. Born in Sodus, he moved to this area 18 years ago from Manassas, Va.
He was a member of the Church of Jesus Christ of Latter-day Saints in DeLand.
Mr. Mullins is survived by his wife, Shirley; one daughter, Sherry Mullins of Alexandria, Va.; three sons, Raymond of Danville, Va., Timothy of Benjamin, Utah, and David of Bothell, Wash.; one brother, Walter Michael of South Carolina; and 11 grandchildren.

Mr. Picallo was an auditor for a private contractor working with several communication companies in the Deltona area. He moved to Deltona in 1995 from Tampa.
He served in the U.S. Air Force.
